What is an FSA plan?
A flexible spending account is an employer-sponsored benefit that allows you to set aside pre-tax dollars to pay for eligible health care and/or dependent care expenses.
Who is eligible to participate in an FSA plan?
Benefits eligible employees that are enrolled in CCT EPO health plan or employees that declined CCT health plan coverage are eligible for the health care FSA. Employees enrolled in the CCT High Deductible Health Plan are NOT eligible. All benefits eligible employees are eligible to participate in the dependent care FSA.
Can I change my annual election amount?
You can change your elected amount during the plan year only if you have a change in employment status or family status (e.g., marriage, divorce, or the birth of a child). Election changes must be made within 30 days of the event. A new employee annual election is required for the health care FSA and dependent care FSA. If a new annual election is not completed employee contributions will end on the last pay period of the current plan year.
Are there contribution limits?
For health care FSAs, the minimum is $100 and the maximum amount is $2,550 for the current plan year. For dependent care FSAs, the minimum is $100 and the maximum is $5,000, including any amount set aside by a spouse into their dependent care FSA.
How can I receive FSA claim reimbursement?
A debit card is available for a $1.50 monthly fee that you may use to pay for eligible health care expenses. If you send a manual FSA claim, the required documentation is:
- For the health care FSA, an explanation of benefits (EOB) that you received for the medical, dental, vision services or pharmacy receipt for which you are filing a claim;
- For dependent care expenses, a copy of the bill or signed receipt is acceptable. Also, have your provider complete the Dependent Care section of the claim form.
